Full Time Case Manager Job Description


Mission Statement: Through the collaboration of local faith and civic groups, Family Promise of Gainesville provides families experiencing homelessness with compassionate care that includes case management, education, housing, meals and transportation until they can transition to a home of their own.
Case Manager position:  Full time position, 40 hours per week.
The Case Manager will provide housing stabilization and help grow our shelter and transitional housing programs. The Case manager reports to the Lead Case Manager. Duties include, but are not limited to:
Intake Specialist

1.   Administered phone intakes daily to determine potential guest and clients for shelter program.

2.   Complete filing and paperwork as needed for intake and grant compliance using the Homeless Management Information System (HMIS) and maintain high data quality standards.

3.   Remain compassionate and truly listen to persons experiencing the trauma of homelessness, to provide resources and referrals to area social service providers.

Shelter Case Management
Volunteer relations are a critical part of this position, as the shelter is 100% volunteer-supported with weeknight dinners and weekend activities for children in shelter. There is room for increasing volunteer programming and mentorship programs.

4.   Assimilate new families into the program (day center tour, supplies, review expectations during their stay and transition to their own homes)

5.   Conduct weekly case management meetings with each family, ensure they complete weekly paperwork and reviewing goal progress

6.   Make referrals to local agencies in accordance with established goal plan

7.   Maintain case notes and a hard copy file of other case related information and tracking sheets.

8.   Conduct disciplinary action when needed through verbal warnings, written warnings and termination of program participation if necessary.

9.   Send weekly profiles to next two weeks' volunteer congregation. Ensure all appropriate changes to profile are made/discuss transportation and family leave issue.

10.Case management duties include identifying family needs to get jobs and maintain housing. These include, but are not limited to; assisting with creation of resumes, assisting in job searches, transporting clients to appointment, and assisting with obtaining services such as medical care. Conduct and develop additional trainings as necessary including but not limited to parenting, nutrition and financial management.

11.Attending Family Promise's Key Coordinator Committee meetings every other month.  Volunteer relations are a critical part of this position, as the shelter is 100% volunteer-run every night of the year.

12.Recruiting volunteers from congregations - share ways that faith groups and civic organizations can engage new volunteers, attend volunteer outreach events, connect new volunteer organizations with existing hosts.

13.Communicating policy, procedures and other information to volunteers, ensuring guest confidentiality and addressing any issues concerning congregation volunteers.

14.Complete monthly reporting necessary for local, state, and federal grants that sustain shelter operations.

When a family is ready to graduate into housing:

15.Complete exit interview with guests, provide aftercare/discharge packet.  Review case plan and complete all appropriate tasks. Print for their future review.  Sign after care contract and complete new case plan for aftercare.

16.Assist families in making sure they have everything setup for a successful move to their new home, including landlord negotiations for rental homes.

a.   Provide from 6 months to 1 year of ongoing support and case management to graduating families, using Progressive Engagement. Conduct regularly scheduled home visits with graduating families based on need, availability and client preference, decreasing in frequency as they become more independent.



Additional Requirements:
The Case Manager must be an enthusiastic individual, interested in community networking and relations.  This position must have a person of good judgment, good understanding of employee and client boundaries, and compassion for others.
Education and Experience:

*        Master's degree preferred in areas of social work, mental health counseling, or marriage and family therapy.  Examples of licensure include L.C.S.W., L.M.H.C., or L.M.F.T.  A bachelor's degree in related field is acceptable.

*        Licenses must be active and unrestricted in the state of Florida.

*        Two years of experience in a case management setting desirable.

*        Experience in volunteer management desirable.

Skills:

*        Knowledge of Alachua County social services is desirable, but not required.

*        Networking communication skills, oral and written: ability to proactively cultivate relationships and make presentations to large groups.

*        Planning and organizational skills:  ability to handle multiple tasks simultaneously and experience in working with policies and procedures from regulatory bodies is useful.

*        Leadership skills: ability to make decisions, problem solve, delegate tasks effectively and follow through with responsibilities.

*        Counseling skills: ability and experience in counseling clients in a social service setting.

*        Computer skills:  Ability to use standard computer office applications and Microsoft Office Suite is required.

*        Attention to detail in file and data maintenance. Experience billing Medicaid for case management is desirable, but not required.

Requirements:

*        Current, unrestricted driver's license and access to reliable transportation.

*        Required to carry state mandated vehicle insurance.

*        Family Promise maintains and enforces a drug-free workplace program.  As part of this program, applicants may be required to submit to a drug and/or screening test.

*        A pre-employment background check is required.
