The survey is asking all county legislators, county directors of public health, and other state and community leaders to participate in the Florida Local Opinion Leaders Survey, which will ask about your beliefs towards tobacco use, tobacco product advertising, and local policies.

Your help is voluntary, but very important. All information collected in this survey will be kept confidential to the extent permitted by law. Responses will be included as part of an average score and no one will be able to identify your individual responses. You can complete the survey online or by telephone.
